Please refer to your policy manual or contact human resource personnel for specifics on coverage, co-pay amounts and deductibles.
For all other insurance, contact them to determine if they pay out-of-network providers and at what rate. It is usually at 70%.
The insurance company will want an ICDM-9 diagnosis code and CPT code. For stuttering therapy the ICDM-9 code is 307.0. The CPT evaluation code is 92506 and the CPT therapy code is 92507. For more insurance information for stuttering therapy log onto www.stuttersfa.org/insuranc.htm
For all other speech-language-voice therapies the diagnosis codes will vary. The CPT codes are the same as above.
